IMPORTANT:
TO SCHEDULE A MANDATORY ORIENTATION AND SUCCESSFULLY SUBMIT AN APPLICATION:

1. Fill out and submit a Recruiter Contact Information Form below.
2. A representative will call/email you to schedule an orientation closest to you.
3. Bring as many of the required documents with you to orientation as possible.
4. Applicant for admission must wear gym shoes, blue jeans, and white t-shirt to orientation.
5. Complete an application at orientation.

Required Documents:
a. Social Security Card
b. Either a State ID or IL Driver’s License
c. Immunization records
d. Birth Certificate
e. Completed Mentor Application
f. Health insurance coverage – If on public assistance, ensure that the card number is on the form
g. High school transcript

NOTE: If you are a resident alien, you will need to bring a copy of your visa.
